Passport with at least six months of validity at the time of your request and at least three blank pages.

Two recent, 2" by 2" color photographs, showing a three-quarter frontal profile showing the right side of your face with your right ear visible.

A notarized letter from your employer, in company letterhead, stating the purpose of the trip and taking full financial responsibility.

IMPORTANT: If you employer uses a computerized letterhead, you must provide a photocopy of the occupational license or a photocopy of the articles of incorporation of the company.

A photocopy of your "green card" or the visa that authorizes you to stay in the United States. NOTE: If you are a U.S. citizen you can disregard this step.
