Passport with at least six months of validity at the time of your request and at least three blank pages.

Two recent, 2" by 2" color photographs, showing a three-quarter frontal profile showing the right side of your face with your right ear visible.

Notarized letter from the school, institution or person guaranteeing full economic support while the applicant is studying in Venezuela.

Notarized letter of acceptance issued by the educational institution in Venezuela where the student is enrolled.

An affidavit from the applicant stating that he or she will not seek employment in Venezuela.

A photocopy of your "green card" or the visa that authorizes you to stay in the United States. NOTE: If you are a U.S. citizen you can disregard this step.
